Chiropractor Initial License Application

Chiropractic licenses expire August 1st of even-numbered years. Pay by check or money order to: Licensure Unit. Your cancelled check is your proof of payment. Payment is processed upon receipt. Debit or credit card is not accepted.
Initial License Fee Waiver:
Beginning January 1, 2020, the initial licensure fee will be waived for applicants of licenses issued under the Uniform Credentialing Act who are:
1) A Young Worker (between the ages of 18 – 25 (or, 17 – 25 if licensed under the Cosmetology Practice Act));
2) Military Family (active duty service members, military spouses, honorably discharged veterans, spouses of honorably discharged veterans, and un-remarried surviving spouses of deceased service members); and,
3) Low-Income (enrolled in a state or federal public assistant program like SNAP, TANF, etc or whose household adjusted gross income is below 130% of the federal income poverty guideline).

The practice of chiropractic is defined as being one or a combination of the following, without the use of drugs or surgery:
1) The diagnosis and analysis of the living human body for the purpose of detecting ailments, disorders, and disease by the use of diagnostic X-ray, physical and clinical examination, and routine procedures including urine analysis; or
2) The science and art of treating human ailments, disorders, and disease by locating and removing any interference with the transmission and expression of nerve energy in the human body by chiropractic adjustment, chiropractic physiotherapy, and the use of exercise, nutrition, dietary guidance, and colonic irrigation. The use of X-rays beyond the axial skeleton shall be solely for diagnostic purposes and shall not expand the practice of chiropractic to include the treatment of human ailments, disorders and disease not permitted when the use of X-rays was limited to the axial skeleton.
On or before August 1 of even-numbered years, each chiropractor holding an active Nebraska license must have completed at least 36 hours of approve continuing education during the preceding 24-month period. The Department waives continuing competency requirements for individuals who were first credentialed within the 24-month period immediately preceding the renewal date.

Claims Made
A policy providing coverage that is triggered when a claim is made against the insured during the policy period, regardless of when the wrongful act that gave rise to the claim took place. (The one exception is when a retroactive date is applicable to a claims-made policy.)
Occurrence
An occurrence policy covers claims resulting from an injury or another event that occurs during the policy term. Coverage depends on the timing of the event. A claims-made policy covers claims that are made during the policy period.
Tail Coverage
An addition to a claims-made policy. It extends coverage for incidents that happened during the time you had your policy, but a claim was not filed until after your policy expired or was canceled. Tail coverage is another name for an extended reporting period.
Note: $1 Million/$3 Million is the minimum plan required in order to participate with some Insurance Companies.

Medicaid Participation
Covered services are only for the treatment of spinal subluxations for which treatment provides a direct therapeutic benefit, and is subject to the following limitations:
(i) For clients age 21 and older, chiropractic treatment is limited to those treatments deemed medically necessary;
(ii) For clients age 20 and younger, chiropractic treatment is limited to those treatments deemed medically necessary; and
(iii) No more than one treatment per client per day is covered.
